New Patient Registration

Registrations are welcome from all patients living within our Practice Area which is within a 5 mile radius.

There are three easy steps you must follow which will authorise us to obtain your medical records and enable us to take over your care safely.

Step 1 - Call into the surgery at any time and collect the registration forms at the reception desk.

Step 2 - Complete and return the registration forms alongside the relevant documentation requested to the reception desk at the surgery between 3.00pm and 4.00pm from Tuesday to Friday. 

Step 3 - Once your forms have been signed by the GP you will be asked to book a registration appointment with the practice nurse. Once attended you will then be registered fully with the practice. 

Medical treatment is available from the date of registration.

New Babies

You should register the birth of your new baby immediately with the Registrar of Births. You will then be given a yellow card (Form HS123) which you should complete and give to the reception staff as soon as possible.
